Passed away Monday, November 4, 2013, age 65. No visitation. Services private. In lieu of flowers, memorials may be made to St. Jude Childrens Research Hospita, 501 St. Jude Pl., Memphis, TN 38105-1905, or American Cancer Society, 2808 Reading Rd., Cincinnati, OH 45206.
